Sounds like you've already addressed the usual culprits - the synchro rings, 2nd gear and other worn components.
The only thing left that I can think of are the poppets in the shifter. I've heared many times over the years that these can and do wear and potentially loose their ability to help hold the trans in gear. I tend to think that there is only some limited holding capacity here to begin with but it's a maybe.
